My mother recently suffered a hyperacute ischemic stroke, confirmed by MRI to have occurred within a few hours. This critical window presented an opportunity for life-saving treatment with tissue plasminogen activator (tPA), which is globally recognized as the standard of care for ischemic strokes if administered within 3-4.5 hours of symptom onset. However, due to the negligence of the Dr. VIKRAM this treatment was not provided. Instead of transferring her to a facility equipped to administer tPA, the doctor admitted her to his hospital, which lacked the necessary resources and expertise. Shockingly, he delayed initiating essential secondary treatments like aspirin or antiplatelet therapy (e.g., Plavix) until I questioned him. His justification--that she was at risk of bleeding--ignored established guidelines that balance this risk against the life-saving benefits of these medications in stroke management. The first 24 hours following a stroke are critical for preventing further damage and complications. Yet, no measures were taken to manage her blood pressure or cholesterol immediately, as recommended in global stroke treatment protocols. The doctor's claim that "treatment is the same worldwide" was contradicted by his actions, which fell far short of international standards. Additionally, his admission that he works excessively long hours raises concerns about his ability to provide competent care. While overwork may explain lapses in judgment, it does not excuse them. A responsible professional should ensure adequate staffing or refer patients to specialized facilities when necessary. This experience reflects a broader issue in patient advocacy and accountability in India's healthcare systems. Patients and families must question their care to ensure adherence to evidence-based guidelines--a responsibility that should not fall on them but on the medical professionals entrusted with their lives.
